Job Description :
- Counsel patients for improved health related outcomes at the hospitals, clinics .
- Responsible for achieving set targets for patient enrollment
- Meet patients at regular intervals as advised
- Manage the compliance and adherence of patients enrolled.
- Collate health records related data from consenting patients
- Capture details related to Adverse Drug Reactions reported by patient in pre-defined templates
- Handling patient queries and providing response from pre-defined FAQs
- Provide patient support by coordinating with pharmacists, stockiest and carrying & forwarding agents
